Abstract

Background: NSAIDs, such as celecoxib (Celebrex), have been associated with a high incidence of cardiovascular events, including heart attack. This study investigated whether the NSAID celecoxib in patients with a history of cardiovascular event may be associated with an increased risk of heart attack in their general population, especially for those who have previously had heart attacks. Methods: This study was conducted in a large, well-designed, randomised, double-blind, parallel-group trial of a large number of patients aged 55 years or more who have previously had heart attacks, as well as in healthy volunteers. Primary analysis: Patients were eligible if they were suffering from heart attacks of more than 6 months duration and had a history of heart attacks or any cardiovascular disease. The trial included a group of patients who had a history of cardiovascular events (e.g., angina pectoris, myocardial infarction, myocardial infarction with or without previous heart attacks, stroke, heart failure, coronary artery disease, or coronary artery bypass grafting), and a group of healthy volunteers. The participants were randomly assigned to receive either celecoxib (2 mg/kg) or placebo. All subjects received the same drug at the same time. Results: Among the patients, the highest risk for heart attack was reported for celecoxib (relative risk=2.19; 95% confidence interval 1.29-3.68; P<0.001), and the lowest risk was reported for placebo (relative risk=2.32; 95% confidence interval 1.20-3.40; P<0.001). Patients who had a history of heart attack but did not have a history of heart attack were significantly more likely to be prescribed celecoxib (relative risk=4.47; 95% confidence interval 1.25-11.09; P<0.001). Conclusions: In the general population of patients with a history of cardiovascular event, the risk for heart attack was significantly higher in patients who had a history of cardiovascular disease.

Introduction

Celecoxib is a COX-2 inhibitor used to prevent or reduce the incidence and progression of cardiovascular disease, especially in patients who have recently had a heart attack.

Celecoxib inhibits the COX-2 enzyme, which is responsible for producing prostaglandins that are associated with inflammation and pain.

While the use of NSAIDs has been associated with an increased risk of cardiovascular events, their use in patients with a history of heart attack or other cardiovascular disease, is generally not associated with a higher risk of cardiovascular events than non-users. As a result, a recent study from our lab found that the use of celecoxib in patients with a history of heart attack was associated with an increased risk of cardiovascular events.

In the present study, we aimed to assess whether celecoxib in patients with a history of heart attack and/or previous cardiovascular disease should be used in patients with a history of cardiovascular disease to prevent heart attack and/or heart attacks.

Methods

A prospective, randomised, double-blind, parallel-group study was performed in adults with a history of heart attack or cardiovascular disease. The study population included patients who had been prescribed celecoxib for their cardiovascular disease for at least 6 months before the study. Patients were recruited from a general community and an outpatient clinic with primary care in the following geographical regions: Gujarat, Gujarat State, India; Maharashtra, Maharashtra State, India; Andhra Pradesh, India; Gujarat, Gujarat State, India; Andhra Pradesh, Gujarat State, India; Andhra Pradesh, Maharashtra State, India; Andhra Pradesh, Gujarat State, India; Andhra Pradesh, Maharashtra State, India; Andhra Pradesh, Gujarat State, India; Andhra Pradesh, Gujarat State, India; Andhra Pradesh, Andhra Pradesh, Gujarat State, India; Andhra Pradesh, Andhra Pradesh, Gujarat State, India; Andhra Pradesh, Andhra Pradesh, Gujarat State, India; and All India, India. Participants were enrolled on the basis of a study of the clinical trials of cardiovascular diseases, as well as on the basis of the medical record of the primary care physicians. All study participants signed informed consent.

The primary objective of the study was to assess whether celecoxib was associated with an increased risk of cardiovascular events in the general population. The secondary objective was to assess whether the risk for cardiovascular events was increased in patients who had previously had heart attacks and/or heart disease.

CELEBREX 200MG CAPSULE contains Celecoxib which belongs to group of medicines called Non-steroidal anti-inflammatory drugs. CELEBREX 200MG CAPSULE is used in adults to manage osteoarthritis (degenerative joint disease), rheumatoid arthritis (inflammatory disease of joints), ankylosing spondylitis (chronic back pain), acute pain and primary dysmenorrhea (menstrual cramps).

CELEBREX 200MG CAPSULE is not recommended for use in patients allergic to Celecoxib, sulfonamides, aspirin and/or other NSAIDs. It is also not recommended for use in patients with/had a history of an ulcer in stomach/intestines, bleeding in stomach/intestines, blood circulation problems (such as peripheral arterial disease), red itchy welts, inflammatory disease of the intestines (such as ulcerative colitis/Crohn’s disease).

The medicine is also not indicted for use in patients suffering from lung problems (such as asthma), heart diseases (such as heart failure, recent heart attack, heart block), severe liver disease (such as advanced hepatic insufficiency) and/or kidney disease (such as advanced renal insufficiency).

Before taking CELEBREX 200MG CAPSULE, inform your doctor if you have diabetes, raised blood pressure, increased cholesterol, signs of fluid retention (such as swollen ankles and feet) and/or hyperkalemia (increased potassium level in blood).

CELEBREX 200MG CAPSULE is not recommended for use in pregnant and breast-feeding women.

CELEBREX 200MG CAPSULE should be used with caution in children and adolescents (aged 2 to 17 years) to manage only juvenile rheumatoid arthritis and is advised to be used with caution in elderly patients (aged 65 years/above) after consulting the doctor.

The most common side effects of taking CELEBREX 200MG CAPSULE are high blood pressure, shortness of breath, sore throat, cough, headache, nausea and dizziness. Consult your doctor if any of these symptoms worsen.

CELEBREX 200MG CAPSULE contains Celecoxib which belongs to a group of medications called Non-steroidal Anti-inflammatory Drugs (NSAIDs). Celecoxib is a COX-1 enzyme inhibitor.
